Healthcare remains one of the most critical sectors in any community or country. Different professionals in this field possess adequate skills that guide and empower them to deliver timely and exemplary services to their patients. The selected community setting for this discussion will revolve around hospice care. Community health nurses liaise with family members to provide timely services to patients in a home hospice in an attempt to minimize health costs and improve medical outcomes. The vulnerable population served in the selected setting is that of elderly citizens. The purpose of this paper is to give a detailed analysis of this setting, the interventions Home Health Nurses (HHNs) implement, and the most appropriate organizations to improve the experiences of the targeted citizens.
Community Setting
The community setting for this analysis is a home hospice established to provide adequate care to elderly citizens who do not have access to various medical services, facilities, or resources. The identified vulnerable population is that of the elderly since they are unable to move freely from point A to B. They also lack adequate financial resources to afford the required health care (Golinowska, Groot, Baji, & Pavlova, 2016). The presence of this hospice program supports the delivery of high-quality medical services.
There are HHNs who work in this hospice to provide personalized healthcare. The first one is the treatment of the common illnesses affecting the elderly, such as arthritis, backache, and chronic conditions. The second one is the provision of timely guidelines to improve mobility and engage in the most appropriate exercises (Nies & McEwen, 2019). Finally, HHNs collaborate with family members and relatives to ensure that the beneficiaries improve the qualities of their lives. Such health services have continued to empower many people who used to encounter various difficulties or challenges before.
Health Promotion Nursing Intervention
The nature of the above setting indicates that HHNs can implement various interventions to record admirable outcomes. Within the past two decades, more boomers have been retiring in the United States (Eldercare Workforce Alliance, 2019). These individuals encounter a wide range of health problems, including chronic illnesses and reparatory diseases. The mission of this agency revolves around overcoming most of the medical challenges many elderly citizens face in this country. It brings together likeminded institutions in order to ensure that positive results are recorded in different regions.
The professional issue this organization takes seriously is creating a superior workforce that can ensure that the elderly get personalized, evidence-based, and accessible medical care in a timely manner. This agency goes further to present superior guidelines, concepts, and ideas that guide different practitioners to deliver exemplary services to the elderly (Eldercare Workforce Alliance, 2019). Many HHNs in the selected setting can acquire numerous incentives from this agency and eventually emerge successful in their respective careers.
Summary
The identified community setting is that of hospice care aimed at meeting the health demands of the elderly.
