Overview of Durable Medical Equipment and Healthcare Common Procedure Coding Systems

Healthcare providers use multiple coding systems to designate services and items to facilitate more accurate payment. The purpose of the Durable Medical Equipment Coding System (DMECS) is to provide distributors, manufacturers, and suppliers with assistance in understanding and finding DME coding in the Healthcare Common Procedure Coding System (HCPCS) Level II (Palmetto Government Benefit Administrators [GBA], 2024). The codes found in DMECS are used for billing and sending claims to the Medicare Administrative Contractor (Palmetto GBA, 2024).

The system is constantly available online and allows finding information such as “active HCPCS, modifier, current fee schedule, or rural zip code, view the PDAC or SADMERC Product Classification List (PCL) details, or export the quarterly fee schedule” (Palmetto GBA, 2019, para. 1). Users can search utilizing code, manufacturer, keywords, item name, or ZIP code (Palmetto GBA, 2019). The following search results were obtained during the use of an interactive tool:

  • A4620 – variable concentration mask, in service since 1993; fee information is unavailable.
  • B4157 – formula for inherited metabolic illnesses, in use since 2005, with no information about fee schedules.
  • A6266 – gauze impregnated with fluid other than water, in service since 1997, with no data on fees.
  • L0626 – sagittal control lumbar orthosis, starting service in 2006, no fee details.
  • L3040 – longitudinal foot arch support, valid for service since 1993, with no fee information.

Using a standardized system with codes accessible to all providers and stakeholders associated with DME has significant benefits. It supports the accuracy and transparency of billing information in health care. The codes efficiently helped find the corresponding item when using an interactive tool. However, there was no data on fees and their schedule for the selected codes. When switching from HCPCS code lookup to fee lookup, it was also necessary to re-enter the code and date, which slowed the search. Nevertheless, the system offers significant advantages when used.
